Fund Raising Permission Slip

CEO is expecting that all students will participate in this exciting camping experience.  We also realize that the $80.00 fee can be an obstacle for some families.  Therefore, we are offering students the opportunity to raise money to pay for their trips by using the attached donation form.  The students’ money will be held in an individual account for them until the time comes to pay for the trip.  Families can also use this sheet to pay for a portion of the camping fee and pay the rest out of pocket if they choose.  Please complete and return the lower portion of this form to give permission for your child to request donations. 

I agree to allow my child _______________________________________________ to solicit donations as a fundraiser for the CEO Leadership Academy camping trip to Camp Mattawa.  I agree to be responsible for turning in all monies collected using the name of CEO Leadership Academy and the camping trip as a cause.
